Applicant and guardian
The applicant’s name and age, a parent or guardian’s name, and your contact information.
Sports program
The program you are interested in, its town, and a contact for the program.
Household information
Annual household income and the number of dependents, as requested by the existing application.
A parent or guardian should complete the consent, electronic signature, and date fields. If you are unsure whether your situation or program fits, contact YSEF before submitting.

Applications are reviewed only by YSEF board members. YSEF does not share application information with coaches, sports providers, or other outside organizations. This form is submitted through our website and sent to YSEF for review.
